PATIENT FEES

Patient fees are reviewed annually and changes are generally effective from 1 November each year, in line with changes to Medicare schedule of fees. Our current Schedule of fees is located on notice board in reception.

Please note that patients who fail to attend their appointment without notifying the practice prior to their scheduled appointment may be charged a non attendance fee. This fee will be required to be paid prior to any future appointments being scheduled at the practice.

Patient transfer fees – When you transfer from this practice we are happy to send your new practice a health summary free of charge. If you require your full file to be sent to your new practice there will be a fee, $25 per individual or $30 per family. This is to cover administration and postage costs.

NEW PATIENTS

We are currently taking new patients.

PATIENT INFORMATION

On attendance of your initial visit with us we require some forms to be filled out, if you could please arrive at 10min before your appointment to allow time for this.
Please bring these with you to your initial appointment:

  • New Patient Demographics
  • New Patient Medical Information Form
  • Privacy and Consent Form
  • Patient Information Brochure

Although most problems are best dealt with at an appointment, you may ask to speak with the doctor on the phone. Doctors will return your call, usually at the end of the day, however they will take emergency phone calls as required.

REPEAT PRESCRIPTIONS

Our patients who require repeat prescriptions are able to book a phone consult with their GP for scripts only.

These appointments are strictly for repeat prescriptions so if you know you need other medical advice please book a normal appointment with your doctor.

TEST RESULTS

Please ensure you phone reception for all results. Do not expect reception staff to interpret your results; they are only able to read the comment that the doctor has recorded. Please phone after 11am.

For your privacy/confidentiality we will not discuss results at the front counter, we require you to phone us to obtain these results.

To ensure appropriate scheduling, please advise if you need the following:

You are able to ask for your preferred doctor when requesting an appointment. Our receptionists endeavor to allocate appointments so that you generally see the same doctor.
